Coordinated Service and Support Plan and the Addendum – ISP (Individual Service Plan).  The Case Manager writes the CSSP. Leanne and/or Corey then write the addendum how the CSSP is going to be met. (The ISP data that you are recording helps monitor the CSSP Addendum to see if changes should be made within the scope of the CSSP.)
